Preschool Information Form
Please download and return the form to the Preschool Office.
Alternatively, you may fill out the form on your computer and submit it via email. To do this, please follow the steps below.
- Make sure that you have the latest version of Adobe Reader. You can download it here.
- Download and open the form in Adobe Reader.
- Fill out the form as normal
- To sign the form, you will need to save it to your computer. Name it something descriptive, such as [Your Childs Name] Information.pdf.
- After the form has been saved, click the button labeled Email Form next to the signature field.
- You will be asked if you wish to use Microsoft Outlook or Webmail (Gmail, Yahoo, Hotmail, etc.) If you do not have or are not sure that you have Outlook or a webmail account, simply print the form out and deliver to the Preschool Office.
- If you select Outlook, then a new email message with some text in the body and the filled form attached will appear. Do not edit any of the text. To send the email, click send.
- If you selected webmail, you'll be prompted to add a Gmail, a Yahoo, or an Other email account. Select the appropriate option and fill in the required information. (Please note that you may need to contact your email provider if you are not using Gmail or Yahoo for the appropriate information.)
- A new page should open in your browser that leads to your email's drafts folder. Select the most recent draft, open it, then click send to send it.
